The rebate applies to both hospital and extras cover for Australian citizens and residents; and applies to the extras component only of overseas visitor hospital and extras cover combinations, or packaged cover, for overseas visitors with a Reciprocal Medicare card. It is not means tested.

5. Would you like to know how you may be impacted by the Medicare Levy Surcharge?
The 1% Medicare Levy Surcharge is a levy payable by high income earners who do not have private hospital insurance.
It affects Australian citizens and residents, and overseas visitors who are eligible for a Reciprocal Medicare card and working in Australia – single people with taxable incomes greater than $70,000 per annum; and families (with or without children) with combined taxable incomes greater than $140,000 per annum. The family threshold increases by $1,500 per child, after the first child.
